29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   At the jobsite located at 1949/1951 N. Cody Lane, Goddard KS 67052, employees engaged in residential framing roofing activities with the use of pneumatic nail guns were exposed to struck-by hazards, in that eye protection was not utilized.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    At the jobsite located at 1949/1951 N. Cody Lane, Goddard KS 67052, employees engaged in residential framing activities were exposed to fall hazards of approximately 9 feet  9 inches from the eave to the ground below in that fall protection was not utilized.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   At the jobsite located at 1949/1951 N. Cody Lane, Goddard KS 67052, employees were exposed to fall hazards of  approximately  9 feet 9 inches in that the 10-foot straight ladder was not secured to the top, nor did the ladder have the length to allow for three rungs to be above the eave of the roof.
