29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, 1105 Ohio River Boulevard, Pittsburgh, PA: On or about March 23, 2026 during roofing activities, the employer did not ensure employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 20 feet.

29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer:  a) Job site, 1105 Ohio River Boulevard, Pittsburgh, PA: On or about March 23, 2026 during roofing activities, the employer did not ensure a system of hazard identification and abatement was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, improper electrical cord repairs made with electrical tape, ladders with either illegible or no safety labels and failing to use head protection.
