29 CFR  1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph:  a.) Heartland Restaurant Group, LLC DBA Dunkin' Donuts. On or about 03/26/26 at the address of 639 Main Street, Clarion, PA 16214: An extension cord was used to power the Rinnai hot water tank in the mop sink area.  ABATEMENT CERTIFICATION REQUIRED:  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of the abatement.

29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   a.) Heartland Restaurant Group, LLC DBA Dunkin' Donuts. On or about 03/26/26 at the address of 639 Main Street, Clarion, PA 16214:  Items including, but not limited to, trash bags, Louisville step ladder, broom, and stanchions were stored within 6 inches of the electrical disconnects in the back of the house area.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

On or about 03/26/26 at the address of 639 Main Street, Clarion, PA 16214, Back of the House Area: Items including, but not limited to, stanchions, bags of salt, garbage cans, and boxes were stored in the exit route which reduced the width of the exit route to less than the width of the door. 29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a.) Heartland Restaurant Group, LLC DBA Dunkin' Donuts. On or about 03/26/26 at the address of 639 Main Street, Clarion, PA 16214: Employees were exposed to hazards associated with the Speakman emergency eyewash station not being inspected on a weekly basis. The eyewash station had not been inspected in months.   b.) Heartland Restaurant Group, LLC DBA Dunkin' Donuts. On or about 03/26/26 at the address of 639 Main Street, Clarion, PA 16214: The Speakman emergency eyewash station located in the back storage area of the store did not work properly. The left side of the eyewash station did not have an adequate waterflow.  NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
