29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, 30 Fordham Street, West View, PA: On or about March 24, 2026 during roofing work, the employer did not ensure employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 25 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  a) Job site, 30 Fordham Street, West View, PA: On or about March 24, 2026 during roofing work, the employer did not ensure ladders were properly set up at the job site in that ladders were extended no higher than approximately 1 1/2 feet above the landing area, exposing employees to fall hazards of up to approximately 20 feet.

29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 30 Fordham Street, West View, PA: On or about March 24, 2026 during roofing work, the employer did not ensure a system of hazard identification and correction was implemented at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, working near an uncovered live electrical service drop, not extending ladders to a proper height above landing areas, using a damaged ladder, failing to use eye/face protection while operating nail guns and using ladders that either had illegible warning stickers on them or none at all.
