29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  At the jobsite located at 8813 W 90th Terrace, Overland Park, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 11 feet from the eave of the roof to the ground in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 8813 W 90th Terrace, Overland Park, Kansas, employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 11 feet from the roof to the ground. The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof.
