29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, 935 Howden Road, Avalon, PA: On or about March 27, 2026 during roofing activities, the employer did not ensure employees on the roof used appropriate fall protection systems/devices, exposing them to fall hazards of up to approximately 20 feet.

29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer:   a) Job site, 935 Howden Road, Avalon, PA: On or about March 27, 2026 during roofing activities, the employer did not ensure a system of hazard identification and abatement was implemented at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, housekeeping issues, ramp from yard to dumpster not of adequate width and sitting crooked, ground person not using head protection, an electrical service drop not blanketed and a ladder with illegible labeling.
