29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    A) 42 Woodlawn Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 20 (front) and 25 & 1/2-feet (side and rear) while working on the rood of a house without the use of a complete personal fall arrest, or other fall protection systems, on or about, March 30th, 2026.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 42 Woodlawn Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 16-feet while using an extension ladder to access an upper level of a structure. The extension ladder that they were using was only extended a foot and a half above the landing and not 3-feet above the landing which is required by the manufacturers instructions. The employees were exposed to falls, ladder collapse hazards, on or about, March 30th, 2026.
