29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 42 Woodlawn Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 16-feet while using an extension ladder to access an upper level of a structure. The extension ladder that they were using was only extended a foot and a half above the landing and not 3-feet above the landing which is required by the manufacturers instructions. The employees were exposed to falls, ladder collapse hazards, on or about, March 30th, 2026.
