29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  a) Residential construction located at 5205 Railroad Ave., Flowery Branch, GA 30542:  On or about 03/26/2026,  the employer exposed employees to struck-by hazards while operating a chain saw, in that protective eyewear was not provided while operating these tools engaged in demolition activities.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) Residential construction located at 5205 Railroad Ave., Flowery Branch, GA 30542:  On or about 03/26/2026, the employer exposed an employee to ejection/fall hazards over 10-feet to the lower ground level, in that a fall arrest system was not used by the employee operating a Genie S-40 Aerial Lift engaged in demolition activities.
