29 CFR  1904.32(b)(6): The employer did not keep the OSHA Form 300A or equivalent posted in the workplace until April 30.  (a) Facility, 250 12th Street West, Huntington: On or about March 31, 2026, the employer did not keep the OSHA Form 300A annual summary for calendar year 2025 posted in the workplace from February 1 through April 30.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1904.41(a)(1)(i): The employer's establishment had 20-249 employees during the previous calendar year and is classified in an industry listed in Appendix A to Subpart E of Part 1904, but the employer failed to electronically submit information from the establishment's OSHA Form 300A by March 2nd.  (a) Facility, 250 12th Street West, Huntington: On March 31, 2026, the employer failed to electronically submit OSHA Form 300A data for calendar year 2025 to OSHA by March 2, 2026. The employer is in covered NAICS 423610 having peak employment of approximately 56 in 2025.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
