29 CFR  1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.  On or about March 18, 2026 at a worksite located at 984 Winchester Road, Neenah, WI; an employee working from the Up-Right Telescope ladder/scaffold unit to remove snow from the top of a semi trailer was exposed to a 13 ft. fall hazard when the ladder section became dislodged from the base.

29 CFR  1926.451(g)(4)(iv): When mid-rails are used, they shall be installed at a height approximately midway between the top edge of the guardrail system and the platform surface.  On or about March 18, 2026 at a worksite located at 984 Winchester Road, Neenah, WI; an employee working from the Up-Right Telescope ladder/scaffold unit to remove snow from the top of a semi trailer was exposed to a 13 ft. fall hazard due to the mid-rail chain missing from the scaffold platform.

29 CFR  1926.454(a)(1): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: The nature of any electrical hazards, fall hazards and falling object hazards in the work area:  On or about March 18, 2026, at a worksite located at 984 Winchester Road, Neenah, WI; the employer did not ensure each employee working from an Up-Right Telescope ladder/scaffold unit was trained on the hazards related to scaffolds, including but not limited to, fall hazards and ensuring the scaffold is secured from displacement while in use.

29 CFR  1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about March 18, 2026, at a workplace located at 948 Winchester Street, Neenah, Wisconsin; the employer did not report an employee hospitalization within 24 hours to the Occupational Safety and Health Administration.
