29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Lot 126, Trusses: On or about April 6, 2026, the employer exposed employees to fall hazards of approximately 18 feet to the lower ground level, in that a fall protection system was not provided to employees engaged in framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Lot 126, roof: On or about April 6, 2026, the employer exposed employees to fall hazards of up to 9 feet to the lower level, in that the A-frame ladder provided did not have siderails which extended at least 3 feet to allow for safe access to each level. b) Lot 126, ground: On or about April 6, 2026, the employer exposed employees to fall hazards of up to 9 feet to the lower level, in that the A-frame ladder provided did not have siderails which extended at least 3 feet to allow for safe access to each level. c) Lot 126, ground: On or about April 6, 2026, the employer exposed employees to fall hazards of up to 11 feet to the lower level, in that the extension ladders provided did not have siderails which extended at least 3 feet to allow for safe access to each level.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) Lot 126: On or about April 6, 2026, the employer failed to inspect all electrical extension cords used to provide power to electric tools which employees engaged in framing operations were using, in that the red & black extension cord powering the DeWalt circular saws was damaged. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
