29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 8, 2026, the employer failed to ensure that all ladders used to access to roof extended at least three feet past the eave of the roof while conducting roofing activities on a residential apartment complex located at 1405 Broadway St, Boulder, CO. The top of the ladder used to access the first-floor walkway from the ground was nearly flush with the top of the wall at the access point. The ladders were not secured at their top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladders.
