29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  a) Residential structure located at 285 Traditions Drive, Alpharetta, GA 30004, Roof: On or about 04/09/2026, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that protective eyewear was not provided to employees operating these tools at 130 PSI.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Residential structure located at 285 Traditions Drive, Alpharetta, GA 30004, Roof: On or about 04/09/2026, the employer exposed employees to fall hazards of approximately 10-feet, 7-inches to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities on a 9:12 pitch roof.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) Residential structure located at 285 Traditions Drive, Alpharetta, GA 30004, Roof: On or about 04/09/2026, the employer exposed employees to 10-feet, 7 inch fall hazards while performing roofing operations, in that the employer allowed for the use of rock-climbing type harnesses and not full body harnesses as the connection to the harness was located near the waist and not middle of the wearer's back.
