29 CFR  1926.501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity.  (a) On or about April 13, 2026, at a jobsite located at 2212 West County Line Road, Mequon, WI 530292, the employer had not ensured that the structural integrity of the existing barn structure was safe for employees to perform roofing work on such as tear off and installation. This exposed employees to a fall hazard of greater than 20 feet into the structure below.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  29 CFR 1926.502(d)(15)(i): as part of a complete personal fall arrest system which maintains a safety factor of at least two; and  29 CFR 1926.502(d)(15)(ii): under the supervision of a qualified person.  (a)  On or about April 13, 2026, at a jobsite located at 2212 West County Line Road, Mequon, WI 530292, employer did not ensure the anchorage points were installed correctly exposing employees to greater than 20 foot fall hazard into the structure below.  The fall protection anchors that employees were using were not installed properly, in-that the anchor points only had 2 nails on each side that were secured to the roof.
