29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Dupuis Lumber, LLC, 36190 Turtle Lake Road, Polson, MT: On or about April 9th, 2026, and at times prior, employees were exposed to struck by hazards while operating powered industrial vehicles including, but not limited to; Hyster 50XT forklift and the JLG 6036 rough terrain forklift, to move materials and product. The employer failed to ensure employees were certified to operate a forklift.
