29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  a) Excavation located at 4805 Setting Down Circle, Cumming, GA 30028: On or about 04/17/2026, the employer exposed employees to cave-in/engulfment hazards while working inside an excavation which measured 6 feet, 4 inches deep by 5 feet, 4 inches wide by 30 feet long, in that in that the large 3 to 4ft. high spoil piles were not kept back from the excavated edge at least 2-feet before allowing an employee to engage in water pipe installation inside the excavation.
