29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about April 20, 2026, at the job site located at N74W24187 Overland Road, Sussex WI, an employee was performing clean up on the ground without a hard hat on while other employees were tearing off shingles on the roof above, exposing the employee to a struck-by hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)On or about April 20, 2026, at the jobsite located at N74W24187 Overland Road, Sussex WI 53089, the employer did not ensure employees tearing shingles off the roof on a single story residential house with a fall height of approximately 8 feet and 9 inches were using fall protection, thereby exposing the employees to a fall hazard.  Evo Contractors LLC (has since been changed to Evolution Contractors LLC) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1530376, citation number 1, item number 1 and was affirmed as a final order on June 22, 2021, with respect to a workplace located at S32W31697 Harvest View Drive, Genesee Depot, WI 53204.
