Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.   On or about April 21, 2026, at a facility located at 545 N Pioneer Road, Fond Du Lac, Wisconsin; the employer did not ensure the use of protective gloves, chemical protective clothing, and eye protection while working with cleaning chemicals, exposing employees to irritation of the skin and eyes.

The employer did not select chemical goggles or face shield, chemical resistant clothing, and waterproof boots or equivalent protection for employees to use while washing trucks and trailers.
