29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. On or about April 21, 2026, at Bronson Peak: employees were exposed to 10 feet fall hazards while installing siding on a 5:12 pitch roof, in that, a fall protection system was not used.

On or about April 22, 2026, at Bronson Peak: the employer did not have fall protection training written certification records for employees exposed to fall hazards of 10  feet while installing siding around a second story window on a 5:12 pitch roof.
