29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Location: Job site;  On or about 04/24/2026, the employer allowed employees to work in a hazardous area beneath an active roofing operation without he use of hardhats in the following instances:  a) an employee was observed cleaning the area beneath the active roofing project without a hardhat or other overhead protection.  b) an employee was observed operating a materials hoist at the back of the residence to move bundles of shingles up to the roof and did not have a hardhat or other overhead protection provided.

Location: Job site;  On or about 04/24/2026, the employer allowed an employee to utilize a Stinger CN100B pneumatic nail gun without proper eye/face protection to protect against potential flying objects.

Location: Job site;  On or about 04/24/2026, the employer allowed employees to perform roofing work on the roof of a single-family residential home, approximately 13-feet above the ground below, without utilizing or tying off to any form of fall protection.

29 CFR 1926.552(a)(1): The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.  Location: Job site;  On or about 04/24/2026, the employer allowed employees to utilize an RGC PRO200 materials hoist/laddervader/shingle hoist as a ladder to gain access to the roof of the residence that was approximately 13-feet above the ground below,  exposing them to a fall.
