29 CFR  1926.50(c): A person who has a valid certificate in first-aid training was not available at the worksite to render first-aid:  a) On or about December 9, 2025, on SR 60 approximately 6 miles east of Westgate River Ranch Resort & Rodeo located at 3200 River Ranch Blvd, River Ranch, FL; during pipe laying work, when a medical facility for treatment of injured and ill employees was not available in near proximity, the employer did not have a person or persons trained to render first aid treatment.

29 CFR  1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:  a) On or about December 9, 2025, on SR 60 approximately 6 miles east of Westgate River Ranch Resort & Rodeo located at 3200 River Ranch Blvd, River Ranch, FL the employer exposed employees to engulfment and crushing hazards in that employees were installing storm drains for an expansion of the roadway to include passing lanes within an excavation up to 10 feet deep without adequate shoring protection.

29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  a) On or about December 9, 2025, on SR 60 approximately 6 miles east of Westgate River Ranch Resort & Rodeo located at 3200 River Ranch Blvd, River Ranch, FL the employer exposed employees to struck by and crushing hazards in that loose soil was left unsecured within 2 feet of the excavation edge while storm drains were installed to  expand the roadway for passing lanes.
