29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) Production Area  - On or about May 8, 2026, the employer did not ensure that employees were adequately protected from rotating parts on equipment, such as the Wafios, CNC Spring and Wire Forming Machine, machine #2 2615120,  Wafios, CNC Spring and Wire Forming Machine, machine #2 7914 321, Wafios, CNC Spring and Wire Forming Machine, machine #2 2617 076,   Wafios, CNC Spring and Wire Forming Machine, machine #2 2615 120, Herdon CNC Spring and Wire Forming Machine, model #CSX-35T, serial #3610.       b) Tool Room - On or about May 8, 2026, the employer did not ensure that the rotating chuck on the Logan lathe, machine #1955, was equipped with a chuck guard to prevent injury.    c) Tool Room - On or about May 8, 2026, the employer did not ensure the unused portion on both sides of the blade on the Johnson horizontal metal cutting band saw was guarded .    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which would provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a ) Tool Room - On May 8, 2026, the employer did not ensure that the 32 ounce container containing petroleum distillates, solvent-dewaxed heavy paraffinic, were labeled with the product identifier and hazards of the chemical.   In accordance with 29 CFR 1903.19(d) abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
