29 CFR  1926.453(b)(2)(v):An employee was engaged in facia activities and wore a personal fall arrest harness but a lanyard was not attached to the boom or basket when working from an aerial lift exposing the employee to fall hazards of approximately 40 feet to the sidewalk below.  a- On 05/14/2026 CSHO observed employees engaged in facia activities from the aerial lift basket were exposed to fall hazards of approximately 10ft to the concrete below as personal fall arrest harnesses were not donned. Condition noted on or about 05/14/2026.
