29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about May 21, 2026 at the site located at 5826 W. Walker Street West Allis, WI 53214 the employer did not ensure employees removing shingles from a second story residential home were utilizing fall protection, there by exposing them to a fall hazard of approximately 13 feet 6 inches to the ground below.

Walker Street West Allis, WI 53214, the employer did not ensure that all employees removing shingles on a second story residential home were trained to recognize fall hazards and how to implement fall protection methods, thereby exposing employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about May 21, 2026 at the site located at 5826 W. Walker Street West Allis, WI 53214, the employer did not ensure ladders used by employees to access the roof of a second story residential home was extended three feet above the landing surface, thereby exposing employees to a 13 foot 6 inch fall hazard.
