An employee was driving a forklift. He fell off the machine during a turn, and one of its wheels broke his left foot. He was hospitalized.

An employee was walking in the bay door area when they were struck by a forklift, resulting in a crushing injury to their left foot.

At about 2:30 a.m. on August 30, 2025, a DHL employee was operating a reach truck in a warehouse. The truck drove into the elevated forks of another, parked reach truck, and the employee suffered a broken left tibia. The employee was hospitalized, requiring surgery.

On August 30, 2025, an employee was operating an electric double-pallet jack. He was coming out of the new extension through the separation curtains when his pallet jack struck an electric forklift. The forklift landed on the employee's right foot, fracturing the big toe and second toe.

Around 12:45 PM, a clamp truck operator went to remove a piece of orange strap from a clamp truck that was parked outside of the office. The strap fell into a gap between the frame and the hood of the truck. The strap became caught in the engine's fan and also wrapped around the employee's hand when he went to grab it. The strap was pulled tight against the frame of the clamp truck, causing lacerations and fractures to the employee's hand. The employee was hospitalized and required surgery.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

An employee was driving a forklift in the parking lot. He stopped and exited the machine to help guide a delivery driver who was backing out. As he walked behind the parked forklift, it started rolling and the employee was struck by the forklift and pinned under a tire. The employee was hospitalized with a broken left leg and broken bones in both feet.

An employee was walking around a flatbed that was being unloaded by a forklift. The employee was placing dunnage on the floor while the forklift removed a double-stack pallet of foam. The dunnage from the double stack pallet became entangled with the adjacent pallet, causing the top pallet of the double-stack to fall and strike the employee. The employee sustained a dislocated ankle and fractured lower tibia and was hospitalized.

Two forklift drivers were stopped next to each other and were talking. The injured employee leaned out of their forklift as the other forklift was leaving and they became pinned between the two forklifts. The injured employee was hospitalized with a fractured rib and a lacerated lung.

An employee was operating a stand-up forklift to travel from the warehouse to the breakroom area. The forklift struck the breakroom wall, causing a fracture/crush injury to his left foot.

Employees were moving an electrical box weighing approximately 200 pounds from a forklift onto a pallet. The electrical box was on the forks of the forklift and the forks were tilted down. The forklift pulled back to have the electrical box slide off the forks. On the other end, employees were bracing the electrical box from tipping over. The box then fell forward, the injured employee slipped while evading the box, the box hit his lower leg and torso, and he fell fell backward. The employee was hospitalized with a fractured T12 vertebra.

An employee was turning out of an aisle on a stand-up powered industrial truck (PIT) when a rack guard penetrated the cab of the truck and pinned their foot against the truck. The employee's left heel was crushed and required surgery.

An employee was stepping out of a powered industrial truck (PIT) when they lost balance and fell to the ground, resulting in a fractured pelvis.

An employee was standing between two beams, acting as the spotter for a forklift. When the forks were placed under the beam being moved, the forks contacted the other beam and caused it to tip over onto the employee. The employee was hospitalized with a broken left leg just below the knee.

A forklift backed into an employee, its rear tire striking her right foot. She fell backward to the ground and suffered a broken left ankle and right foot. She was hospitalized.

An employee was operating a forklift to return a scrap hopper after dumping material. After the employee placed the hopper outside the aircraft hangar door, the employee noticed the hopper rolling toward the doors. The employee exited the forklift to stop the hopper and grabbed the top of the hopper. During this time, the forklift started rolling forward and struck the hopper, pinning the employee s right hand between the forklift mast and the hopper and resulting in hand fractures.

An employee was walking to a break area when she was struck and run over by an industrial forklift. The employee was hospitalized with an injury to her body and a severe leg injury.

An employee was driving a forklift to a charging station to park. His left foot was caught between the vehicle and an angle iron barrier. He suffered a cut and a fracture to the foot and was hospitalized, requiring surgery.

An employee was retrieving pallets from a conveyor feed using a stand-up forklift. While turning a corner on the forklift, the tip of his left foot struck a bollard, resulting in fractured toes.

On August 20, 2025, at 8:00 a.m., two employees were removing a suction module on a mud pump. Three bolts were sheared off and the fourth bolt was frozen, making it difficult to remove the module. The employees used a 5-foot pry bar inserted in the bolt hole to free up the module while a forklift bumped and lifted the module using a two-part sling. When the forklift bumped the module, the module came free, causing the pry bar to jump and pinch the injured employee's left little finger against the pulsation dampener, which resulted in a partial fingertip amputation.

The injured employee was operating an unloaded powered industrial truck (PIT) and was following a stacker truck that was carrying a palletized load. The stacker truck stopped and the injured employee's right lower leg impacted the palletized load on the stacker truck, resulting in a fracture.

An employee was walking through a warehouse when they were struck by a forklift, resulting in a fracture, dislocation, and degloving on the right foot.

An employee was operating a forklift on a slope to move a concrete barrier wall. The barrier wall swung due to the rough terrain, causing the forklift to overturn. The employee suffered fractured vertebrae in their back, a fractured shoulder, and a head laceration.
