An employee was descending a fixed ladder on the side of a container loader. The employee fell from the ladder, landing on the ground about 6-8 feet below and sustained fractures to their back and pelvis.

Four employees were traveling in a utility task vehicle (UTV) on the east lot of the site. While turning, the UTV flipped over onto the driver's side. The driver's left wrist and hand were crushed by the frame of the UTV resulting in a fractured and dislocated wrist, as well as hand injuries. The driver was hospitalized and required surgery.

An employee was driving a forklift. He fell off the machine during a turn, and one of its wheels broke his left foot. He was hospitalized.

An employee was walking in the bay door area when they were struck by a forklift, resulting in a crushing injury to their left foot.

At about 2:30 a.m. on August 30, 2025, a DHL employee was operating a reach truck in a warehouse. The truck drove into the elevated forks of another, parked reach truck, and the employee suffered a broken left tibia. The employee was hospitalized, requiring surgery.

On August 30, 2025, an employee was operating an electric double-pallet jack. He was coming out of the new extension through the separation curtains when his pallet jack struck an electric forklift. The forklift landed on the employee's right foot, fracturing the big toe and second toe.

On August 30, 2025, an employee was moving product using a hand truck/dolly. He observed an electric pallet jack obstructing his path and went to relocate it. The electric pallet jack ran over his right ankle, dislocating it.

An employee had been completing a phase of electrical work, installing wire harnesses. At approximately 2:15 p.m., the employee was operating a UTV when it rolled onto its side. The employee sustained fractures to the right wrist and hand.

An employee had been loading a flatbed. Their right foot was run over by a mini telehandler. The employee was hospitalized with a fractured foot.

An employee pulled into the customer's driveway, parked, and exited the vehicle. He opened the sliding side door on the vehicle to retrieve a package. The customer began backing their vehicle out of their garage and it struck the employee in the back of his left leg. He was pinned him between the two vehicles, causing contusions and lacerations.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

There was a dirt clog preventing the tailgate of a dump truck from closing all the way. An employee got out of the driver s seat and walked around back to push up against the tailgate to unclog the area. The tip of his finger was amputated by the tailgate.

At about 7:15 a.m. on August 28, 2025, an employee was using a construction compactor to stabilize the dirt of a dam that had just been built. The dirt gave way underneath the compactor, which tipped on its left side. The employee fell out and was crushed and pinned between the compactor and the dirt. He was hospitalized, having suffered a broken sternum and broken ribs on both sides, as well as a laceration to the left leg by the ankle joint.

An employee was checking for air flow from an automobile's engine fan. The fan caught his left hand and lacerated his index, middle, ring, and little fingers, with a partial amputation of the little finger.

An employee was working to drop the tailgate on a rented dump truck. His right thumb was caught in a pinch point between the tailgate and a retaining chain. He suffered a partial amputation to the thumb, past the last knuckle.

A car was being backed out of a bay so that another car could pull into it. The car's brakes did not respond, and the car reversed quickly. The injured employee approached the car to stop it, and it pinned his right leg against a lift. The employee was hospitalized.

An employee was riding an all-terrain vehicle (ATV) in a field when the ATV tipped over. The employee fell off the vehicle and sustained a fractured hip.

An employee was driving on an active construction site when their vehicle struck a concrete paving machine. The employee impacted the steering wheel and sustained fractures to both arms.

An employee was working to unload cargo from a ship. A steel coil contacted the employee's left leg. The lower leg was fractured.

An employee was walking around a flatbed that was being unloaded by a forklift. The employee was placing dunnage on the floor while the forklift removed a double-stack pallet of foam. The dunnage from the double stack pallet became entangled with the adjacent pallet, causing the top pallet of the double-stack to fall and strike the employee. The employee sustained a dislocated ankle and fractured lower tibia and was hospitalized.

An employee placed an aluminum step ladder beside a tractor trailer and was removing a piece of scrap aluminum from the trailer that resulted from a motor vehicle accident. While coming down the ladder, the employee fell approximately 6 feet to the concrete. The employee was hospitalized with a fractured T4 vertebra.

Two forklift drivers were stopped next to each other and were talking. The injured employee leaned out of their forklift as the other forklift was leaving and they became pinned between the two forklifts. The injured employee was hospitalized with a fractured rib and a lacerated lung.

An employee was descending a dirt road on a utility task vehicle (UTV) after finishing fueling a booster pump. The vehicle left the dirt road and went over a fallen tree. The employee sustained broken ankles, a sprained left wrist, and a contusion/scrape on his upper abdomen. The employee was hospitalized.

An employee was on the floor of an aisle wrapping a pallet when an electric pallet jack struck him, resulting in fractured ankles.

An employee was taking a scissor lift off a flatbed trailer. While using the remote operator controls and walking alongside the lift, the wheel of the lift turned in the wrong direction, causing it to strike the employee and roll over his left ankle. The employee was hospitalized with two broken bones in his ankle.
