An employee was helping to remove attachment forks from a loader. The attachment failed to come off. The employee went to shake the attachment as the loader lowered the forks. The employee's left index finger became caught under the attachment, resulting in a fingertip amputation.

An employee was on horseback in a pasture checking on the health of calves. The employee roped a calf when their right-hand fingers got caught in the rope, resulting in amputations of three fingers at the top knuckles that required hospitalization.

An employee was cleaning around the rear part of a peanut washing operation. He was repositioning a shovel when the handle slipped from his hands, and his left hand and right thumb were caught in a chain sprocket on the conveyor. His right thumb was amputated and he sustained lacerations to his left hand. The chains and sprockets were unguarded.

An employee was adjusting a rubber drive band on a tractor when the band shifted and pinched their finger against a rotating part. The top of the finger was amputated.

Employees were pushing a potato conveyor belt as one employee was adjusting the belt to make it shorter. The injured employee's right index and little fingers were crushed by a roller and a metal plate. The fingers were partially amputated and required sutures.

An employee was grating sweet potatoes on a harvester when their right ring finger got caught in a belted chain on the harvester, resulting in a fingertip amputation.

Multiple employees were working to treat a bloated animal in a cattle pen. They put a head restraint on the animal. A chain attached to the head restraint device caught the injured employee's index finger, resulting in a fingertip amputation around the fingernail area that required surgical amputation at the first knuckle.

Three employees were transferring a metal lid using a manual chain hoist to place it onto a cart. While they were adjusting its position, the lid shifted and the injured employee's left hand became caught between the hoist hook and the handle, resulting in a laceration and amputation of their ring fingertip including the tuft.

Two employees were operating a wood splitter. An employee was loading wood into the splitter when the tip of his right thumb was crushed between the rear end of the wood and the back metal plate of the wood splitter. The employee's thumb tip was amputated.

An employee was making rounds to the barns to take care of cattle. The employee was stepping out of a van and the sliding door closed on their little finger. The employee's fingertip was amputated.

On July 23, 2025, at 8:00 a.m., an employee was power washing. He went to retrieve a power-washing tip from the machine's fan while it was in operation and the fan blade contacted his right index finger, severing it at the distal joint.

An employee was using an 8-foot metal pipe to remove a manure blockage at the transfer to an incline conveyer. The employee s left hand was placed on top of the pipe while removing the blockage. The pipe pushed their left hand into the wooden railing of the platform they were standing on, catching their thumb in a pinch point and resulting in partial amputation.

An employee was using a chainsaw to cut brush during wildfire suppression operations and line preparation. The chainsaw bar became stuck in brush while he was cutting limbs off a juniper tree. He reached to move the brush and his left-hand fingers contacted the moving chain. The employee's index and middle fingers were partially amputated, and he sustained severe tissue damage to his ring finger that was reattached.

An employee's fingertip was crushed between a hydraulic clamping device and a post, resulting in an amputation.

An employee was removing a pin from a metal gate to let cows out of a parlor. The gate moved and his left middle finger was caught in the pin hole. The employee's finger was partially amputated.

An employee was inside an elevator when the elevator malfunctioned and fell to the ground, causing his body to go upward. His right arm got caught between the elevator and its frame, resulting in an amputation of the arm.

An employee was setting a roof truss when their thumb became trapped between the roof truss and the steel plate. The distal phalanx of the thumb was surgically amputated.

An employee was pulling two separate egg racks when the wheels of the front rack locked, causing the second rack to crush his thumb between both of the racks. The employee sustained an amputation to the tip of the right thumb.

An employee was installing a feed roller. The employee's left index finger was caught between the roller and a choker. The employee suffered an avulsion to the finger, and the fingertip was partially amputated at the hospital.

An employee was clearing the last of the debris from a manure truck bed when his right hand was caught in the floor chain. While he tried to free his right hand, his left arm was also caught. Both of the employee's arms were amputated.

An employee was demonstrating a tenderizer machine to another employee. The machine caught the employee's left index finger, causing fractures. Most of the finger was medically amputated. The incident occurred during a training activity.

An employee was removing plant flats from a pulley conveyor and was adjusting plants on the conveyor. The employee put their left hand under a flat to fix it, and their glove got caught on a pulley, resulting in amputation of the left little finger.
