An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

An employee was cutting wood with a circular saw. The machine lagged and cut the employee's finger. The employee required surgical amputation of the finger at the hospital.

An employee was reading numbers off a belt that feeds a plant. He reached his hand up and the belt caught his finger, resulting in a finger amputation.

An employee was standing on an 8-foot A-frame ladder to pull multiple Category 6 cables for cameras. As he reached for the cabling, the ladder tipped over and he fell approximately 6 feet. The employee suffered a fractured left tibia just above his ankle, as well as multiple fractures in the ankle.

An employee was working on repositioning nitrogen tanks/bottles on storage racking when their left thumb was crushed by the storage racking. The tip of the thumb was amputated.

An employee was using the pipe-and-wrench method to loosen a seized bolt on a roof truss. A technician from another company was on the other side of the truss doing the same action with the same method. The employee's socket slipped off the bolt and his left hand was crushed against the truss, resulting in injuries to the index and little fingers, requiring hospitalization, surgery, and amputation of the index finger.

An employee was adjusting the forks on a skid steer when his right index finger got pinched between the forks and the frame of the skid steer. The finger was crushed and required partial amputation of the fingernail area.

An employee was cutting a PVC pipe when the saw slipped, amputating his left ring fingertip. The fingertip was reattached.

An employee was helping to install a marble plank when the plank fell on his hand. His fingers became caught between the plank and the floor, resulting in partial amputations to two fingers.

An employee was using a metal shear when it amputated the tip of his right index finger.

Two employees were preparing 10-foot angle iron pieces (galvanized steel) to be placed on top of bridge beam edges. The north end of the angle iron dislodged and fell, which caused the south end of the angle iron to twist and pinch an employee's right middle fingertip against the beam edge. The employee's fingertip was amputated.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was kneeling on the floor, looking through material under a desk that was next to a metal exterior door. A gust of wind blew the door closed, and his left little finger was caught in its frame. The fingertip was amputated.

An employee was assisting with pre-welding setup on pile structures at the fuel dock worksite. They were working to operate a come-along to align a waler to the pile. Due to limited leverage and restricted positioning within the man basket of the crane, the employee exited the basket to manually adjust the waler from the structure itself. During the task, wave height and frequency increased. A large wave struck the area, causing the pile braces and walers to shift. The employee lost their footing and grabbed the production pile edge. Their fingers on one hand were crushed between two structural steel piles. The employee sustained an amputation to the fingers and was hospitalized.

An employee was assisting three coworkers with loading a fall protection metal storage cage onto a wooden pallet. Their right index finger was pinched between the pallet and the bottom of the cage, resulting in a fingertip amputation.

Employees were working under a platform walkway, removing bollard pipe. An employee went to lift and get the bollard unstuck from the footer. The bollard came free and shifted into the corner of the adjacent wall, causing their right ring finger to get caught between the pipe edge and wall corner. The employee sustained an amputation of the distal knuckle of the finger.
