An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.

An employee was using a bucket truck hoist to raise secondary aerial wiring. The wire made contact with the primary wire, causing an arc flash. The employee suffered burns to both hands and was hospitalized.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

Employees were securing the fuel line of an outboard motor in preparation for removing the motor from a small vessel located in the equipment yard. A gasoline-related fire occurred and one employee sustained burns to both hands and forearms.

An employee was climbing down a 6-foot ladder. The employee slipped at the last step and fell to the ground, suffering a broken femur.

Two employees were wrapping a porch column with plywood. One employee was in an aerial lift and the injured employee was on the ground below, cutting plywood. An employee was descending from the roof of a one-story residential home onto a ladder when he fell from the ladder to the concrete below, resulting in a broken left heel and right ankle.

An employee was on an extension ladder descending from a roof when the ladder collapsed and lowered. The employee fell approximately 20 feet to the ground, resulting in fractures to their ribs and left wrist.

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

An employee was kneeling on the floor, looking through material under a desk that was next to a metal exterior door. A gust of wind blew the door closed, and his left little finger was caught in its frame. The fingertip was amputated.

An employee was operating a utility task vehicle (UTV). During a left turn, the vehicle rolled over on its right side. The employee suffered a lumbar strain and fractures to four lower-back vertebrae. She was hospitalized.

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

An employee was working on the second floor of a construction site. They were removing a piece of plywood that was placed over a 2-by-2-foot hole in the floor when they backed up and fell through the hole to the first floor below. The employee landed on the cement flooring and suffered a broken wrist.

An employee was underwater, hand-dredging loose material from the bottom of a water-filled pump vault. While ascending from a depth of 30 feet (on a non-decompression profile), the employee sustained an over-pressurization injury to their lungs.

An employee was walking up to a truck and walked into the open lid of a toolbox. The employee sustained a fracture to the corner of their right eye.
