An employee was diagnosing the cause of a rear light malfunction on a forklift when the reverse gear engaged. The forklift moved backward and contacted the employee causing a metatarsal fracture in their left foot and injury to their lower right leg.

An employee working at the reject conveyor was placing a product guide bar on the conveyor. This metal guide bar is placed on the side of the conveyor belt to funnel the product, preventing it from falling off the sides. As he was getting the attachment positioned, his left ring finger was caught between the conveyor belt and the conveyor belt frame. The employee's finger was fractured and partially amputated.

A mechanic was performing a modular overhaul on a blower. While removing magnetic rods for the stretch rod, the employee was placing the second rod on the blower platform. The rods were magnetically attracted to one another, and the employee's left third and fourth fingers were pinched between them. She was hospitalized and sustained a partial amputation.

An employee had been using a tub dumper to dump product on a belt. The employee was letting the tub dumper down and removing the tub when his right little finger was crushed in the frame and amputated.

On August 25, 2025, a machine operator was working on the heat-set end of a textile bleach/prep range machine. He was running leader cloth and it was bunching up. He climbed a catwalk so he could reach the cloth and flatten it out. His right hand was caught in a nip roll while adjusting the cloth. The employee's middle and ring fingertips were amputated.

An employee was repositioning the de-panner infeed belt on the oven. Their left ring finger was caught in the belt resulting in a laceration, nail loss, and partial amputation.

An employee was working on a degerminator, which had been lifted by a crane. The machine fell onto the employee's arm and broke it.

An employee was cleaning off a pulley on a conveyor belt when the conveyor caught her right index finger, resulting in a fingertip amputation.

An employee was moving vats when the vats fell and struck his left ankle, catching it against a small guardrail and breaking it.

An employee was working from a ladder and adjusting a spreader guide roller with a T-handle Allen wrench. The employee came into contact with an electrical junction box/wires, was shocked, and fell off the ladder to the concrete floor. He suffered a broken right-side rib, a puncture in the right lung, and an electrical shock burn to the right palm.

An employee was inserting corn tortillas into a corn cutting machine. The machine's air piston released and came down onto her left thumb, resulting in an amputation.

An employee slowed his forklift in a warehouse aisle as another forklift turned from an intersecting aisle. The employee's left arm was pinned between the two forklifts. The employee developed compartment syndrome.

An employee was operating a battery-operated pallet jack and went to open the gate for the dry storage elevator. The pallet jack continued moving and struck their right lower leg, fracturing it.

On July 31, 2025, two employees were walking near a friction wash tank when a rubber gasket on top of the tank blew. Hot water mixed with 50% caustic chemical sprayed out and rained down on the employees. Both employees were brought to an emergency shower and were neutralized with vinegar. One employee was hospitalized with chemical and thermal burns to their upper back and eyes.

On July 7, 2025, an employee was cleaning food from under a conveyor belt. He was retrieving a piece of food when his left middle finger contacted the chain and pulley. The fingertip was amputated. The machine was guarded at the time of the incident.

An employee was removing a tub of ground chicken and scrapping off the grinding tube when their finger was caught in the grinder. The employee sustained an amputation and required surgery.

On May 27, 2025, an employee had been sweeping the floor around the sort and grind line conveyor. She was wiping down the frame of the conveyor when her right arm was drawn up between the conveyor belt and the snub roller underneath the conveyor. She sustained fractures to her upper and lower arm.

On May 19, 2025, a maintenance technician was repairing a conveyor belt on a packaging machine in the beef production room. The employee pointed at a chain drive and the tip of his left glove was pulled into the chain, resulting in partial amputation of his left index finger.

An employee was changing the belt of a sorting machine. He was on his knees, changing the drive belt under the machine, when lost his balance and hit his head on the framework. He suffered an injury that caused numbness in both arms and hands and was hospitalized.

While explaining to a co-worker how to install a new gate on the palletizer machine, an employee's left thumb tip was amputated by the machine's piston.

An employee was retrieving a pallet using a forklift. The employee stepped off the forklift to adjust a box. Before the employee returned to their own forklift, they were struck by a second forklift as it backed up. The employee's lower left leg was pinned between a pallet (carried by the second forklift) and the cab of their own forklift. They suffered multiple fractures to the lower leg.

An employee went to clean out a back-up of potatoes on a potato recycling conveyor and inspect the line to see what was causing the issue. When removing potatoes that had become lodged in the sprocket end, their right arm was caught in the sprocket, causing a fracture of the distal radial shaft of their forearm. The employee was hospitalized and required surgery.

An employee was performing maintenance on an air actuator and was working to clear a jam on a ball valve when the ball valve amputated their left index fingertip.

An employee was adjusting the belt for a breader machine when the machine's lid came down on their left hand, resulting in a partial amputation to the middle finger and hospitalization.

An employee was changing a hydraulic cylinder from a poultry-washing machine. During the installation of the bath, a 4-by-4 filler block broke, causing the top plate to come down and strike the employee's left hand. The employee suffered amputations to the index and middle fingers.
