An employee was using a meat saw to slice pork when the blade amputated their right little fingertip.

An employee was cutting hams on a band saw. As he reached around the back of the saw to take hold of a piece of ham, the saw blade cut his left middle finger. The finger was medically amputated at the middle knuckle.

An employee was clearing a jam on a production line. A machine cycled; its auger rotated and amputated the employee's left index fingertip.

An employee was working on a cushion ring (steel). As they lifted the cushion ring with both hands to flip it up, the ring slid down and the employee's right middle fingertip was crushed between the ring and the work table. The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

An employee was building wood detail blocking and a wood member needed to be resized. The employee took the wood member to the table saw and set the fence to the correct size. The employee pressed down on the board to control it when his left little finger contacted the running blade, resulting in an amputation.

An employee was cutting floral stem bunches. She was grabbing a piece of plastic out of the stem cutting machine when the machine cut and amputated her left index and middle fingers.

An employee was processing a pork loin when a larger slab of meat fell off the cutting surface. While the employee went to catch it, the sawblade amputated the employee's left little finger.

An employee was performing rebar fabrication using a bending machine. The employee's glove became caught in the area where the rebar bend overlaps onto itself. The subsequent bending/cutting caused the two overlapping portions to squeeze together and amputate approximately 1/4" of his left index fingertip.

An employee was working underneath a vehicle when the vehicle began to roll and a tire caught the employee's left ear. The employee sustained a laceration and partial amputation of their ear, requiring hospitalization and surgery to reattach the ear.

An employee was clearing a pipe along a culvert using a motorized plumbing machine. The end of the machine got tangled and stuck in long grass and other debris. The employee went to lift the end of the machine out of the grass and clear the blockage when his hand was pulled into the machine, resulting in a partial amputation and open fracture of his left distal phalanx.

An employee was installing a driveshaft on a customer's vehicle. He was using a floor jack to hold the driveshaft in place to line up the bolts. The driveshaft moved off the jack and crushed his left ring fingertip, resulting in an open fracture of the tuft of the distal phalanx, as well as a partial amputation.

An employee was moving a toolbox from a pallet to the sales floor when his left middle finger got caught between the toolbox and the concrete floor, resulting in a fingertip amputation of the distal digit.

An employee was operating a meat slicer when their fingertip was amputated by the slicer blade.

An employee was operating a panel saw machine when the machine cycled and pinched his left middle fingertip, resulting in a fingertip amputation.

An employee was cutting a 15-pound piece of ribeye on a saw. The blade of the saw contacted the employee's right hand causing a partial amputation to the middle finger and injury to the index finger requiring stitches.

An employee was working to fix film in a roll stock machine while it was in operation. The die box pinched and amputated his right middle fingertip. The machine was guarded at the time of the incident.

An employee was delivering building supplies to a job site. As he was pushing a cart through a door, the cart tipped. The employee went to catch the cart and his right ring finger was caught between the cart and the door jamb, resulting in a partial amputation.

An employee was retrieving equipment from the mop room. Their hand was in the door hinge and when the door closed, the employee's finger was amputated.

Three bakery mixer bowls were stacked on a rolling transportation cart next to bags of flour, adjacent to a starter fermenter. An employee was moving bags of flour. Needing to move the bowls, the employee moved the cart. The bowls tumbled off the cart and bounced off the floor. While the employee was trying to stop the bowls from rolling, his right ring fingertip was crushed between two of them. He suffered a contusion and amputation to the fingertip.

An employee was raising an electric ladder to retrieve merchandise from overhead racking. As it rose, the railing of the machine caught his right middle finger against the bottom of the high bay netting supports, causing an amputation just below the fingernail.

An employee was cutting floor tile with a power saw. The saw kicked back and the blade came down on his left hand, lacerating the index finger and amputating the thumb tip.

An employee was operating a router when it cut his right little finger, causing an amputation through the bone.

An employee put a till in a safe and closed the safe door when her finger was caught in the door, resulting in a partial fingertip amputation.

An employee was using a photo cutting trimmer board to trim photos and amputated their left thumb tip without bone loss.
