An employee was rinsing a food processing machine at a facility and went to stabilize himself. His left hand contacted the surface of the machine and a guard inadvertently closed, crushing the tip of his ring finger and resulting in an amputation and fracture.

An employee was using a table saw to cut a piece of wood when the saw cut his thumb, index, and middle fingers. His index finger was amputated and he was hospitalized.

A temporary employee was helping set up a stage for a concert. The employee was run over by a mobile cart and sustained a toe amputation. The employee was hospitalized.

An employee was performing maintenance on a hopper. The employee's hand was caught in a cylinder pivot point area when the pneumatic cylinder released trapped air. The cylinder crushed the employee's left little finger, causing the partial amputation of the fingertip.

An employee was walking behind the rewind coding machine while on his phone. He rested his left wrist on top of the slitter bar and his finger came in contact with the roller, resulting in amputation.

An employee was helping to repair an accident-damaged guiderail along the highway. The repair involved installing two embedded tubes in the ground and connecting them with a strut, then connecting the tubes to wooden breakaway posts. The employee was holding a wooden breakaway post while a coworker was using a post driver truck to move one of the embedded tubes to align with the strut and breakaway post. The tower of the post driver contacted the employee's right middle finger, resulting in an open fracture to the distal phalanx and a traumatic amputation to the fingertip.

An employee was pulling out the winch rope on a hand-fed chipper with a winch. The rope was tightened and it caught his left hand, partially amputating his index fingertip and lacerating his index, middle and ring fingers.

An employee was loading a skid steer rock rake onto a trailer for transportation and was securing it. The rock rake shifted and crushed their fingers, resulting in amputations to two fingers.

An employee was on a ladder, adjusting the upper limit switch for an ash expeller. He slipped off the ladder and his left ring finger was caught between the expeller piping and the moving ram cylinder, resulting in a finger fracture and an amputated fingertip.

The injured employee was trimming a bush with a hedge trimmer. He reached over with his left hand to remove a branch. The blades of the trimmer partially amputated his left index finger.

A temporary employee was filling a sweeper with gas at a construction site. He was shocked by the jumper cables, which caused him to move his right hand into the sweeper's rotating fan blades. His middle fingertip was amputated.

An employee was pushing a lawn mower away from bank of a lake. He went to prevent it from falling when the blades contacted three fingertips on his right hand, resulting in amputations.

An employee was working on the cylinder of a railroad truck to repair a hydraulic leak. The blade was being kept in place by a 4-by-4-inch piece of wood while he pulled a lever to force the blade flush against the block to secure it for the inspection. The employee sustained an amputation to the left thumb tip.

At 11:15 on September 8, 2025, an employee's hand was caught and injured by a gear as the employee was cleaning out a gearbox. His left middle finger was injured, and he underwent a medical amputation to the fingertip.

A temporary employee had been stacking parts and then went to another work area. He accessed and operated a hydraulic correction machine. While adjusting a part, his left little finger was caught by the correction ram, resulting in amputation of the distal portion of the finger above the fingernail.

On August 27, 2025, an employee was using a push mower when the discharge flap became pushed up by grass. The employee let go of the mower handle to stop the motor before reaching under the mower. The blades were still spinning and lacerated multiple fingers. One finger was partially amputated.

An employee was reaching to pull a door closed when her right middle finger got caught in the door, resulting in a fingertip amputation.

An employee was using a rag to grease the chain on a salt spreader when their right middle finger was caught by the chain, resulting in a partial amputation and a laceration.

An employee was training to use an industrial shredding machine. While he was dumping a box, the pulley on a conveyor caught the tongue of his right shoe and pulled his third toe into a pinch point between the conveyor belt and the pulley drive. The employee sustained an amputation to their toe.

An employee was unloading a plate tamper from a pickup truck. The employee's right little finger was pinched between the tamper's handle and its plate, resulting in a partial amputation at the nailbed.

At about 7:40-8:00 a.m. on August 11, 2025, an employee was operating a riding mower near a ditch and fence line with heavy weed growth. The blades of the mower became entangled in vines and stopped abruptly. The employee was removing the vines when the mover moved slightly and its blades partially amputated the employee's left middle finger.

An employee was mowing a common area at a shopping center using a stand-on mower when the mower hit a rut and he fell off. His right hand made contact with the mower deck's blades as he fell, resulting in an amputation of the middle finger and an injury to the thumb that required hospitalization.

An employee was cutting pieces of a conveyor belt when a piece became caught on the saw. The employee sustained lacerations to their left index and middle fingers, resulting in amputation of the middle fingertip.

On July 29, 2025, at approximately 8:30 AM, a temporary employee was performing the riveting process at her assigned station on the stamping line. Her left index fingertip became caught in a pinch point of a stamping machine. The employee's fingertip was partially amputated.

On July 28, 2025, at approximately 2:40 p.m., an employee was inspecting and cleaning a hopper when their left middle finger was pinched by a pneumatic actuated feed block, resulting in a fingertip amputation.
