An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was installing a light fixture. While descending a 6-foot ladder, they missed the third rung and fell to the ground. The employee's tibia and fibula were fractured.

An employee was coming from the parking lot at the start of his shift. He fell while walking in the crosswalk and sustained pain and numbness in his right leg. The employee was hospitalized and required surgery.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

The employee then spun the belt and motor/drive assembly by hand to ensure that the belt was seated properly, and that the motor/drive assembly was in proper alignment. His right little finger became trapped between the belt and motor/drive assembly, resulting in partial amputation of the distal aspect.

An employee was preparing for services in the chapel when he tripped on a stair/step and fell. The employee sustained a fracture to their distal femur with intra-articular extension and hemarthrosis.

An employee was walking to her work area when she tripped over another employee's foot and fell. She was hospitalized with a fractured hip and required surgery.

At 12:45 a.m. on September 21, 2025, an employee was working to clear an obstruction in a machine when his left hand came into contact with a turning screw inside the machine. His index and middle fingers were amputated.

An employee was on a ladder cutting a piece of metal off the fire escape of an abutting building. The employee lost their balance or their foot slipped, causing them to fall approximately 7 feet to the ground. They landed on their feet, but fell backward, hitting their back on the corner of the concrete foundation. They were hospitalized with a laceration to their back that required surgery.

An employee was inspecting the blower section of an HVAC unit when the wheel shaft hooked onto his sweatshirt and pulled his arm into the unit. The employee suffered a puncture wound and a broken bone in his arm, resulting in hospitalization.

An employee was helping a forklift operator wrap hoisting straps of a tote of chickpeas onto the extension blades of the forklift in order to unload the tote from a railcar. The employee was putting the back-left strap on the extensions when the forklift moved inward his left little finger got caught between the fork and a wall, resulting in a fingertip amputation.
