An employee was walking in a garage toward her delivery vehicle. As she stepped down a curb near her vehicle, she fell and landed on the concrete garage floor, resulting in a hip injury that required hospitalization.

Employees were performing maintenance operations on a railroad line when the machine experienced a load conveyor motor failure. While clearing the machine to allow for repairs, an employee pinned the left side of the swing conveyor. The operator on the right side then requested a slight bump to the left to align the right side for pinning. Once the swing was in position, the operator went to pin the right side and the chain tensioner failed, resulting in an abrupt release of the swing chain. The chain snapped back and struck the operator s right hand, resulting in a 1-inch laceration and a fractured fifth metacarpal. The employee was hospitalized and required surgery on the hand.

At about 9:15 a.m. on March 10, 2025, an employee was riding along a hardpack snow-covered roadway on a snowmobile. The snowmobile went over a berm and tipped/rolled, causing the employee to fall to the ground. He suffered a collapsed left lung and was hospitalized.

An employee was decommissioning a substation. The tower switch was being set down on the ground when the switch blade moved. The employee's finger was crushed and amputated.

An employee was struck by a truck and sustained a broken right leg.

On September 9, 2024, an employee was operating the dual pass rough cull (DPRC) machine when their left index finger contacted the chain belt, resulting in amputation of the fingertip.

On August 7, 2024, an employee was unloading jersey barriers from a forklift when his right middle finger was caught between a piece of dunnage and the asphalt and was partially amputated.

On July 15, 2024, at 5:15 p.m., an employee was delivering mail on a property when they were bitten by a dog, resulting in hospitalization due to puncture wounds and a broken bone in the left wrist.

On March 17, 2024, at approximately 11:50 p.m., an employee was operating a ride-on electric pallet jack on the loading dock after loading a truck when he was struck by an all-purpose container (APC) that had fallen off the forks of another electric pallet jack that was being operated nearby. The APC struck the employee in the lower left leg, resulting in a fracture and hospitalization.

On October 24, 2023, an employee suffered a heart attack while completing physical fitness testing on a designated obstacle course.

On October 4, 2023, an employee was performing a walkaround of the grounds when they fell off the edge of a sidewalk onto their shoulder. The employee suffered a left humerus fracture and dislocation, requiring hospitalization and surgery.

An employee was stocking items on a shelf in a bent position. When the employee stood up, she experienced lower back pain and mobility difficulties due to a lumbar back strain.

On April 12, 2023, at 12:30 PM, an employee was cutting a wood block on a table saw. A piece of the block fell into the saw and pulled the employee's left index finger in with it, resulting in a fingertip amputation.

On April 2, 2023, at 4:30 AM, an employee was on his way to start his vehicle in the parking lot when he slipped and fell hitting his head on the asphalt. The employee sustained brain bleeding/swelling requiring surgery.

On March 22, 2023, at 12:30 p.m., an employee was returning to her vehicle after delivering a package when she slipped on ice and fell to the ground, resulting in a fractured right ankle.

At 10:10 a.m. on March 21, 2022, an employee was loading bubble wrap onto a flatbed truck from a dock. The employee fell between the truck and the dock, landing on the concrete pad below and suffering a thoracic spinal fracture.

On March 17, 2022, an employee was riding a bicycle between maintenance locations. In maneuvering to avoid a pedestrian, the employee fell, landing on the floor and on a radio he had been carrying on his left side. He suffered a broken left hip.

An employee fell from a boom and landed about 7 feet below on his upper torso. He suffered a right-side orbital fracture, a kidney laceration, and a wrist fracture.

A crew was lifting a bridge with bottle jacks so they could remove and replace floor-to-ceiling joists. Soil at one end gave way, causing a wall of lumber to collapse on the foreman who was observing them. The foreman suffered fractured vertebrae.

An employee was clearing snow out of a parking area, using a mini-loader with a bucket attached. The bucket hit a water shutoff valve sticking up above the road surface; the mini-loader stopped, and the employee hit its glass front door and shattered the glass. The employee suffered a head laceration, an injured tongue, an injured sternum, broken ribs, and an injured hand.

An employee was unloading freight from a semi-trailer. He missed a step getting off the trailer, fell, and hit his head, suffering a scalp laceration and dizziness. He was hospitalized.

On October 25, 2021, an employee parked at a residence to deliver a package. As the employee exited the vehicle and reached into the driver's side to remove the keys, the front driver's side tire rolled back on his leg, resulting in leg fractures.

On May 28, 2021, an employee was cutting onions with a standard chef's knife and partially amputated their left thumb.

On May 12, 2021, an employee was attending quarterly firearms training when her 9mm discharged into her right thigh and exited out of her lower right leg.

An employee was cutting product on a shear machine when the machine amputated his right thumb.
