An employee was fixing an issue with seed cleaning equipment when he slipped on the ladder he was on. He went to catch himself and his hand went into a moving belt and pulley on the cleaner. The employee sustained the amputation of his right middle and index fingertips just below the fingernails.

An employee was riding an all-terrain vehicle (ATV) to move a bison herd. He was riding up the side of a steep hill when the ATV struck a rock and rolled, ejecting the employee before rolling over him. The employee suffered broken vertebrae in his back that required hospitalization.

An employee was clearing out dirt from an underground pipe when he was bitten by a rattlesnake on his hand.

An employee was climbing in the spreader area to remove a pasta jam when his right little finger was cut by the rotary cut-off knife. The employee sustained a laceration and fracture, resulting in an amputation.

A smoke jumper parachuted out of a helicopter during a forest fire response, landed on steep rugged terrain, and suffered femur and rib fractures and a liver laceration. The employee was hospitalized.

An employee was riding a horse and leading several pack mules along a trail. A few mules started acting up, causing the horse to stir. The employee then fell off the horse and landed on a stiff woody protrusion that impacted her chest/rib area. The employee was hospitalized with a fractured rib and a punctured lung.

An employee was working on a tree thinning unit and was cutting a 6" diameter tree. During the back-cut when the chainsaw kicked back. The saw struck her, resulting in a 3-inch laceration on her left leg below the knee. She was hospitalized and required surgery.

An employee was moving a steel frame by hand when his right middle finger was pinched between the frame and a steel saw horse. The employee sustained a crushing injury to the finger and the removal of skin that required surgery and hospitalization.

An employee was coaching a bike lesson. Their handlebar clipped a tree, rotated, and struck their left side. This caused the employee to fall, and they were hospitalized with a ruptured spleen.

An agricultural tire failed and struck an employee's face and left hand. The employee suffered a cut above the left eye and multiple fractures in the hand and was hospitalized for surgery.

An employee was transferring hot oil from one containment tank to another. The employee removed a dust cap when hot oil came out the top and splashed on the employee, resulting in multiple third-degree burns to their neck, arms, and other parts of the body.

Employees were loading a set of grain hoppers. A lid blew shut on the next car to be loaded. While flipping the hopper lid back open, an employee fell from the top of the rail car to the ground 14 feet below. He suffered two compound fractures to one leg and a fracture to the other leg/ankle.

An employee was performing a proficiency jump during smokejumper training. Increasing wind and turbulent air caused the employee to have a hard landing; the employee suffered a fractured femur, a fractured right foot, and compression fractures to two lower-back vertebrae. The employee was hospitalized.

Two employees were putting a broken pallet jack into a metal recycling bin using a dock stocker. The jack became stuck on a pallet of totes next to the bin, causing the jack handle to snap back. It pinched the injured employee's fingers against the bin, resulting in an amputation to the ring fingertip.

An employee was opening a tailgate on a customer's truck when it unlatched and he fell to the ground. He struck his neck on the truck's trailer ball hitch as he fell, resulting in dizziness, nausea, and discomfort. The employee was hospitalized with a neck contusion.

An employee was standing on a standup mower and reversing it when the mower spun around. The employee jumped off the mower and it struck and ran over his right foot, amputating his third, fourth, and fifth toes.

During a maintenance day, two operators locked out the lathe to replace the trash gate cylinder. To complete the lockout, the lathe operator raised the trash gate to the pin position while another operator inserted a pin for the gate to rest on while the work is performed. When this event occurred, the lathe operator was in the cab, and the other operator was at the trash gate with the blocking pin. While inserting the pin, the gate rose from the run position to the pin position, pinching the injured employee's left middle finger and resulting in a fingertip amputation.

An employee was forming a light pole foundation when a cable anchor failed, causing the concrete form to fall and pin the employee between the form and the wall. The employee was hospitalized with a fractured pelvis.

An employee was on the ground unloading fertilizer when he tripped and fell over a cinder block that was on the ground. The employee was hospitalized with a fractured left hip.

An employee was on the rig floor pulling rod. A bolt sheered off the strong arm which came down and struck the employee in the chest. The employee was hospitalized for a fractured sternum and internal injuries to the spleen and colon.

An employee was under the rig floor adjusting a chain on the slips when his finger became caught between the chain and the blowout preventer (BOP). The employee sustained the amputation of his finger.

An employee was cutting pork with a band saw when his left hand was caught by the saw and the index and middle fingers were amputated. A guard was in place at the time.

An employee was walking into a customer's building when he slipped on ice and fell, resulting in fractures to his foot.

An employee was cutting a 2x4 to build a safety rail when he dropped an 8-inch section. The wood fell about 5 feet and struck the injured employee's face. The injured employee required surgery on his eye.

A large laser table weighing 1500 lbs. was being moved into a storage unit for customer. Wood decking was placed on the floor of the storage unit so the table was not sitting directly on the concrete floor. As the laser table was set down, one employee's left index finger was caught between the bottom of the table and the decking, resulting in amputation below the first knuckle.
