An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was removing counter flashing from a roof when his hand slipped and struck the counter flashing. He sustained a laceration to his right hand and a severed tendon. He was hospitalized, and surgery was required.

An employee fell 5 feet from a retaining wall to the ground. The employee was hospitalized with three fractures.

An employee was clearing a jam on a ring press. The press's secondary switch was activated, engaging it. It caught the employee's hand. The employee was hospitalized and sustained the amputation of two fingers. The machine was unguarded at the time of the incident.

On October 14, 2025, an employee was on a step platform showing another employee the support bracket for a blow pin assembly on a blow molding machine that needed repaired. The blow pin assembly came up and pinched the injured employee's right middle finger, resulting in a broken finger and a laceration to the top of the hand.

An employee was on a reach truck staging pallets. Another forklift struck the employee's lift, throwing him off and pinning his right leg against a pallet. His lower leg was fractured and he was hospitalized.

Two employees were moving a railcar (plastic pellet hopper car) using a mobile railcar mover. One employee was the engineer in control of the railcar mover and the injured employee was a switchman. The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

At about 9:20 a.m. on October 10, 2025, an employee was backing up a stand-up forklift in a storage warehouse to move pallets of goods from steel pallet storage racks. His right knee was pinned between the forklift and the steel racking, and his femur was broken near the knee. He was hospitalized, requiring surgery.

An employee fell to the floor while moving a helium tank. The tank fell onto the employee, who sustained a fractured left wrist, a laceration to the left hand, and an impact injury to the head.

An employee was correcting a bend on a brake press when their left middle finger was crushed in the machine.

An employee was cutting material on a straight saw when the blade lacerated her right index finger. The employee was hospitalized.

An employee was taking trash to the dumpster outside when he lost his balance and fell on the stairs. He strained his neck and sustained bruises on his back.

An employee was cleaning a machine and bleeding its lines. After the gauge read 0, the employee removed the metal lid covering a filter. The lid popped off and struck the employee in the face, causing a broken nose, a broken jaw, and lacerations.

An employee was registering a patient in police custody in an emergency department. The patient tried to escape, and police/security personnel bumped into the employee during the resulting scuffle. The employee fell and suffered a broken right tibia and fibula.

An employee was shadowing coworkers while in training on the wide extrusion line. The employee went to clean the roller with a rag and cleaner when his right index finger got caught between a powered roller and a rubber roller. The employee sustained an amputation to a finger.

An employee was cleaning the restroom when she slipped and fell to the ground, injuring her ribs on the right side. The employee was hospitalized.

An employee was standing on a trailer. The employee lost balance and fell partially off the trailer, resulting in fractures to both ankles. The employee was hospitalized.

An employee was washing drip trays at a sink. She then carried multiple drip trays to the beverage area. She tripped over a box of trash bags and fell against a refrigerator. She sustained a fractured sacrum.

An employee was operating a lathe. Just before the end of his shift, he used a towel to wipe off a part in the machine and his finger was pulled into the machine. He sustained a partial amputation of the right fifth distal phalanx.

A truck driver was preparing a hose to load a chemical tanker trailer with sulfuric acid when sulfuric acid splashed the left side of their face and neck, causing skin burns.
