An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.

An employee was struck by a plasma cutter and suffered a broken left tibia and fibula. The employee was hospitalized.

An employee tripped on a rotor on the floor of a store, fell, and suffered a broken right thumb.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee was using a hose to clear storm drains after a sewer backup. The employee fell backward and his head struck the curb, resulting in an Injury on the right side of his head, generalized bruising, and pain in his left ankle, right hip, left shoulder, and neck. The employee was hospitalized.

An employee was mixing self-leveling cement when his hand was caught in a mixer. He suffered a partial amputation to his left middle finger, as well as a laceration to his buttocks.

An employee was walking through a passageway sideways when the edge of her shoe got caught and she fell onto her right hip, resulting in a broken femoral head.

An employee was working on a roof. He fell from the roof area onto a concrete pool slab about 30 feet below, striking his head. He suffered a severe laceration to his forehead and a fracture to the right arm.

An employee was offloading tools from a van and walking them over to the curb. He lost his footing and fell while carrying a jackhammer. The jackhammer fell and caught the employee's right little finger against the curb. The employee's finger was amputated.

An employee was using a roofing nail gun when it fired a nail into his left knee. He was hospitalized, requiring surgery.

An employee was removing stretch wrap from between shipping pallets when she tripped/stumbled and fell, resulting in a fractured hip.

An employee was walking through a remote location on the worksite looking for spare parts. The employee was stung by a bee, suffered a severe reaction, and was hospitalized.

An employee strained his back after reaching for product on a pallet. The employee was hospitalized.

A bartender tasted a small amount of an unknown liquid found behind a bar. He had a severe reaction to the liquid, which proved to be concentrated liquid detergent, and he suffered burns to the interior of the mouth. He was hospitalized and required surgery.

An employee was cutting a piece of concrete curb with a cut-off saw when it kicked back and lacerated their left arm.

An employee was adding cilantro to the dicing machine when it became jammed. While clearing the jam, the blade amputated approximately 1/2" to 3/4" of his right middle finger.

A resident pushed a walker in front of an employee, causing her to trip and fall. The employee struck her head on the floor and was hospitalized with a brain bleed.

An employee was working on a garbage truck when they experienced heat illness, resulting in a hospitalization.

An employee was cutting a metal square on a hydraulic shear when his left hand got caught between the metal and the shear block. The employee's middle finger was amputated.

An employee was loading a gaylord into a truck when the gaylord ripped, causing him to lose his balance and fall from the liftgate to the pavement. The employee was hospitalized with fractures to his left wrist and femur.

An employee was working product in the back of his truck to prepare for a delivery when he tripped over a pallet and fell to the floor of his trailer. As he fell, he struck his head on the side wall of the trailer, resulting in head/neck injuries.

An employee was installing an automobile lift at a client location and put the lift on a safety lock. The employee's right hand was caught in the lift and crushed, causing the amputation of the little finger at the last knuckle and all of his right fingers were fractured.

An employee was climbing an interior fixed ladder to access the roof when they slipped off the ladder rung and fell approximately 10 feet. The employee sustained injuries to both ankles.

An employee tripped and fell over a U-boat while putting supplies from the U-boat into the pharmacy. The employee was hospitalized for a dislocated shoulder and required surgery.

An employee fell to the floor while walking and broke a hip.
