An employee was entering a building. When he grabbed the door handle with his left hand, the suction from inside the building started to close the door, causing him to lose his grip. He went to catch the door with his right hand before it closed when his middle fingertip was caught and amputated.

An employee was finishing disassembly of an engine block, guiding the camshaft through the bore of the cambox of a naval ship's engine. The employee was standing on the rear platform of the engine rotisserie stand when his right foot slipped and he fell in between the stand and the engine. He planted his foot in the oil drain pan and then slipped again, hitting his back against the frame of the stand. The employee sustained a fractured right scapula, two fractured ribs on the right side, and bruising to the right lung.

An employee was unloading pallets and empty wooden crates from a truck using a forklift and a portable ramp when the forklift hit the inside of the ramp wall at the entrance to the ramp. The forklift tipped over and the employee suffered an injury to their right knee and calf. The employee was hospitalized.
