On June 23, 2025, at approximately 8:50 a.m., an employee was working in the wash bay area, using an overhead crane to move galvanized steel formwork panels that weighed approximately 386 pounds. The overhead crane was rigged with transport bolts, shackles, and an O-ring. While the employee was lowering one of the panels into position, the shackles and O-ring were caught on the edge of the panel. During final positioning, one of the shackles dislodged and the panel shifted suddenly, causing the employee's fingers to be caught between two panels. The employee's left middle fingertip was amputated, and they sustained a laceration on their left index finger.
