On February 26, 2025, maintenance team was called to replace a bearing on the overarm on a slitter. At approximately 4:24 PM, employees were utilizing the designated rolling platform (approximately 2 feet above the ground) to access the overarm (approximately 6-7 feet above the ground). An employee stepped down off the platform and climbed onto the slitter machine. They slipped and fell approximately 3 feet to the concrete ground below, landing on their right elbow. The employee was hospitalized with an open fracture to the right distal humerus.

On April 29, 2024, at around 8:45 AM, an employee was making a fuel delivery to a 300-gallon fuel tank that was located on top of pea gravel at a customer's site. The gravel started to shift during the delivery and the employee jumped from their step ladder about 24-30 inches to the gravel to avoid the tank and then fell. The employee sustained a laceration to their left eyebrow and three broken ribs on their left side.
