An employee went to remove the air bar from a roll of plastic sheeting and needed to unjam a shaft that was stuck inside a metal core. He stood on one side of the shaft and core, and went to push/pull the shaft out of the core in order to free the core. The shaft became free all at once, pulling the core toward the employee; his fingers were pinched between the core and the shaft. His ring fingers on both hands sustained cuts and fractures and required stitches. His little fingertip was fractured and required surgical amputation.

An employee was cleaning the cylinder on a press when their left hand was pulled under the cylinder and injured. The employee was hospitalized.

Berry Global Incorporated
The injured employee was standing near a plastic roll machine while another employee was taking a quality control sample of the plastic film. The machine was powered off at the time of sampling. Upon completion of the sampling, the machine was started up again and a metal shaft was ejected. The metal shaft still had a roll of plastic on it; it weighed approximately 920 pounds and it was positioned approximately 8 feet above the ground. The metal shaft struck the injured employee, who then fell into the second employee. The second employee fell to the ground and sustained contusions but was not hospitalized. The injured employee's left leg was broken below the knee and the employee was hospitalized.

An employee was cleaning an area around a conveyor section in the tempering department when their left thumb contacted a drive shaft with a pulley. The employee was hospitalized and their left thumb was partially amputated.

An employee was building foam tubs and stacking them on a pallet. As she was stacking a tub on the pallet, her left foot caught the pallet and she fell to the floor. She suffered a broken left femur and was hospitalized.
