An employee was dumping cardboard boxes into a recycling container using a forklift. When he tilted the forks to allow the boxes to slide off into the container, the pallet also slid into the container. While attempting to remove the pallet, the employee slipped and fell, hitting his face on the ledge of the container. He continued to fall to the floor, hitting his face again on the pavement. He broke two eye sockets, his nose, and a bone behind his upper lip.
