An employee was standing on an 8-foot A-frame ladder to pull multiple Category 6 cables for cameras. As he reached for the cabling, the ladder tipped over and he fell approximately 6 feet. The employee suffered a fractured left tibia just above his ankle, as well as multiple fractures in the ankle.

Two employees were changing fuses for a fryer. The pump on the fryer was turned off, but the heat exchanger kept running. When the pump was turned back on, hot oil shot toward the filter. The filter box opened and sprayed the two injured employees with hot grease. The employees suffered burns to their backs, shoulders, and arms.

Tumlinson Electric
Employees were standing outside of a manhole and pulling a rope through an underground conduit using a telehandler. They were using mule tape tied to the electrical line at one end and the telehandler at the other. The employee noticed the mule tape was slipping off the forks of the telehandler and he grabbed it. The mule tape wrapped around his left index finger and constricted, resulting in amputation of the finger.

An employee was working under a pickup truck, changing out the shift linkage cable. The cable was being attached to the gear shift from within the cab. When the cable was pulled within the cab to connect it, the truck went into neutral and rolled over the employee's left shoulder. The employee suffered fractures to the left humerus, left shoulder blade, and left-side ribs, as well as severe contusions to the left shoulder.

An employee was removing burning material from a machine. While being used to extinguish a fire, the diffuser/horn of a fire extinguisher disconnected from the hose and the hose struck the employee's neck, back, side, and shoulder. The employee sustained a closed fracture of thyroid cartilage and a vertebral artery occlusion.
