Ogden Air Logistics Complex
An employee was servicing equipment and had disconnected a 4-inch line. A proportional control air valve on the equipment actuated, releasing a large volume of air at 99 psi into the 4-inch braided stainless steel line hanging down from the valve. The sudden release of pressure caused the line to whip and strike the employee's face. He was hospitalized due to an orbital bone fracture requiring surgery. The equipment was not locked out/tagged out at the time.

An employee and a colleague were moving two pieces of rebar when a tie was cut. The employee holding the rebar dropped it while it was above the injured employee's hands, resulting in partial amputation to their left middle finger.

An employee was vacuuming heavy metals in the attic of a building when he fell through a sheet rock ceiling and landed on the floor approximately 30 feet below. The employee was hospitalized with lower back injuries.

Atec Inc
Two employees were assembling a roll-up door. Tension had been applied to help roll the curtain into the door barrel; one of the employees was using a metal bar and a pin to secure the pre-tension wheel as this tension was slowly released. The pin dislodged and the wheel spun, pinching the employee's left hand between the end bell metal plate and the bar. The employee's index and middle fingers were crushed and the tip of the left index finger was amputated.

Helix Electric, Inc.
An employee had been connecting cables between a load bank and a transformer (neither were connected to an energy source). After that he began to roll out and position cables to connect the transformer to an electrical switchboard. While positioning cables at the rear of the switchboard in preparation for making the final connections, he contacted an energized buss bar. The employee sustained first degree burns on both arms, with second degree burns on his forearms and hands. He also sustained first and second degree burns on the left side of his chest. Additionally, he sustained two exit wounds on his left leg, one on the knee and the other just above the knee.

An employee was walking with a steel pipe. The employee tripped over a portable bandsaw and fell; the sharp end of the pipe struck the employee's hand, amputating about 3/4 inch of the right little finger.
