When the employee opened the jaws to remove the tricone, his left thumb was caught between the backside of the jaws and the receiver. The tip of the thumb was amputated just above the nailbed.

An employee was inside a building on a 6-foot stepladder, using a chain come-along to draw the building together at the top so a bolt could be installed to hold it together. The chain released, swung back, and struck the employee's head, knocking him off the ladder. He suffered a skull fracture.
