On March 16, 2015, at 4:15 P.M., an employee was working at the charge chute for polyester reactors. The employee had used a hoist to lift and dump an approximately 2,000-pound bag of PET pellets into a hopper that was connected to the reactor. He started to walk over the pallet on the floor toward the empty bag. When he stepped off of the pallet, he lost his balance and landed awkwardly on his left foot.
