An employee fell into an active excavation. The crew was setting trench boxes while the employee stood beside one of them. As the operator attempted to position the box, it dropped approximately 3 feet, causing the employee to lose his balance and fall into the excavation. The employee sustained serious injuries: a dislocated shoulder, a small chip in a bone, and a slight crack in his pelvis.
