Final narrative
An employee was working on a paper plate machine. The machine jammed; it stopped feeding paper, but was still running. The employee opened the safety gate on the cutting die to stop the machine. When the employee opened the safety gate, the safety switch engaged to de-energize the machine. He reached into the cutting die, and his left-hand middle finger was pinched between two parts of the cutting die. He was hospitalized, and his fingertip was later medically amputated.
