On September 28, 2015, at approximately 2:45 p.m., an employee was climbing a 28-foot extension ladder that had been leaned against a mid-span, which is the line between two poles. The ladder began twisting and moving, and the employee fell 15 feet to the rocky ground below. He fractured his left wrist and required surgery.
