Final narrative
An employee noticed that a plastic bottle of shampoo had fallen over on the conveyor belt because it was missing the cap. The employee thought he had hit the stop button to shutdown the conveyor belt in order to grab the fallen shampoo bottle, but he didn't thereby his right index finger got caught between the chain and the sprocket. This resulted in the amputation of the fleshy tip of his right index finger.
